When treating patients with dental prosthetics and implants—including crowns, bridges, dentures, and implants—orthodontists must possess a sophisticated understanding of how to manage clear aligner therapy. Although it is possible to treat such conditions with clear aligners, these factors introduce additional complexities into the treatment process. It is crucial to note any of these conditions on your prescription form when planning treatment for a patient with these prosthetics.

 

Complexities Introduced by Implants & Prosthetics

• Bridges: These may create undercuts that complicate the insertion or removal of aligners due to excessive retention.
• Dentures: Designed to align with the opposing arch, any movement in the teeth can necessitate adjustments or replacements of the dentures.
• Partial Dentures: Likely to require replacement as moving teeth within an arch that includes a partial denture can lead to poor fit. If treatment with clear aligners is considered for a patient with a partial denture, impressions or intraoral scans should be taken without the partial denture. It is recommended that the partial denture not be worn during treatment to avoid interference with tooth movements.
• Implants: Since implants are fused to the bone, they cannot be moved. Ensure that all implants are clearly marked on your prescription form to prevent any planned movement of the implanted teeth, which could jeopardize the implant.
